Drug-resistant epilepsy: when brain surgery becomes the best option

Living with seizures that medication cannot control is exhausting and limiting. For some patients, brain surgery is not a last resort but the best option available.

Most people with epilepsy control their seizures with medication. But roughly one in three does not respond well enough, and for them there are options many do not know about, including surgery.

What is drug-resistant epilepsy?

It is epilepsy in which seizures continue despite properly using at least two well-tolerated antiseizure medications at the right doses. It is also called refractory epilepsy. Looking for alternatives is necessary, because uncontrolled seizures affect safety, memory, mood and quality of life.

Is epilepsy surgery a last resort?

No. For many patients it is the option most likely to greatly reduce seizures or even stop them. To consider it, the area of the brain where the seizures start, called the epileptogenic zone, must be identified precisely.

How is it decided whether someone is a candidate for surgery?

With a presurgical evaluation that combines several studies:

  1. High-resolution MRI, to find lesions that cause the seizures, such as dysplasias, hippocampal sclerosis or scars.
  2. Video-EEG, which records brain activity and the seizures at the same time to locate the focus.
  3. In some cases, PET or functional brain mapping, to outline the area and protect regions for language or movement.

What are the results of surgery?

When there is a clear focus in an area that can be operated on without affecting critical functions, the results can be very good. In temporal lobe epilepsy with hippocampal sclerosis, the most common form, a significant proportion of patients become seizure-free.

What options are there if surgery is not possible?

When a single focus cannot be identified, or it is in an area that cannot be operated on, there are alternatives such as vagus nerve stimulation, deep brain stimulation or radiosurgery.

When should I ask for a specialized evaluation?

If you have had poorly controlled seizures for more than two years despite treatment, an evaluation at a specialized epilepsy center is worthwhile. That step can completely change your situation.

In summary

Drug-resistant epilepsy has more options than many patients know, and surgery can be one of the best.
